Youth Faith Formation Registration

- Class time and day: Tuesday 6:00-7:30 PM - Confirmation Year 1 and Confirmation Prep/Confirmation (7th and 8th grade)
  Tuesday 6:00-7:30 PM - Grade 1-6
  Wednesday 4:30-6:00 PM Grade 1-6
  
  Tuition: One child $300 Two Children $400 Three or more Children $500
  
  Sacrament Fees: First Eucharist/Reconciliation- $60 Confirmation Year 1 - $80 Confirmation Year 2- $150
  
  Eucharist fee is due for the students that are making their First Holy Communion this year.
  
  Confirmation Year 1 is for students in 7th grade or above who wish to prepare for the Sacrament of Confirmation.
  
  Confirmation Year 2 is for students who have attended either St. John's School or public school students who have completed Confirmation Year 1 Prep. These students will be attending additional classes once a month on a Sunday and will attend a Retreat.
  
  Tuition and fees are required at time of registration. Families with outstanding balances from previous year must complete payment before registering. No one will be denied Faith Formation due to the inability to pay. If you need financial assistance, please contact the Director of Lifelong Faith Formation, Mary Thon at the Parish Office.

  MEDICAL AUTHORIZATION FOR TREATMENT
  In the event that the undersigned, or my authorized physician, cannot be reached and in the judgement of a responsible St. John the Evangelist staff member, there is a necessity for immediate examination and/or treatment of my child/children enrolled in St. John the Evangelist's Youth Faith Formation Program, I hereby request and authorize any of the aforementioned personnel to obtain for my (our) child/children such medical services as are deemed necessary.
